Numerous large tophi with associated joint deformity are clearly appreciated in both hands of this patient.

•Gout is a form of inflammatory arthritis characterized by recurrent attacks of a red, tender, hot, and swollen joints, where crystals in the form of monosodium urate form inside and around joints (called tophi).

•The most known hallmark of the disease is hyperuricemia, but by itself it does not indicate gout. Hyperuricemia means an increased production of uric acid due either a defect or hyperactivity in an enzyme, HGPRT (Hypoxanthine Guanine Phosphoribosyl Transferase) and increased cell turnover (or cell breakdown) in patients that use chemotherapy for cancer, have chronic hemolysis, or have hematologic malignancies.

•Another even more common reason for gout is decreased excretion of uric acid by the kidneys due to renal disease and NSAID or diuretics usage.

•Inflammation typically develops when uric acid collect in the synovial fluid as the extracellular fluid becomes saturated with the acid which is then phagocytized by polymorphonuclear cells. This process develops inflammation.

•Pain typically comes on rapidly in less than 12 hours.

•The joint at the base of the big toe is affected in about half of cases. It may also result in kidney stones, or urate nephropathy.

•Severe gout is characterised by frequent polyarticular flares, numerous tophi, joint damage, and musculoskeletal disability.
This is a preventable condition and in many cases, represents a disease that has been insufficiently managed for years.

TREATMENT:
Standard management recommendations may be insufficient for patients with severe gout. Such patients frequently require intensive individualised pharmacological management with combinations of Urate-lowering therapy, Steroids, Colchicine and Anti-inflammatory agents.

The upper 2 syringes are containing dark red blood drawn from a vein, while the bottom 3 syringes contain more reddish and brighter blood taken from an arterial source.
Arterial blood is the oxygenated blood in the circulatory system found in the pulmonary vein, the left chambers of the heart, and in the arteries. It is bright red in color, while venous blood is dark red in color (but looks purple through the translucent skin).

Venous blood is deoxygenated (lower in oxygen and has higher concentration of carbon dioxide, giving it a dark colour) blood that flows from tiny capillary blood vessels within the tissues into progressively larger veins to the right side of the heart.

It is then pumped by the right ventricle to the lungs via the pulmonary artery which is divided in two branches, going to the left and right lungs. Blood is oxygenated in the lungs and returns to the left atrium through the pulmonary veins.

Venous blood is the specimen of choice for most routine laboratory tests, obtained by direct puncture to a vein, most often located in the antecubital area of the arm or the back (top) of the hand. It is typically colder than arterial blood, and has a lower oxygen content and pH. It also has lower concentrations of glucose and other nutrients, and has higher concentrations of urea and other waste products.
The most common reason for collection of arterial blood is the evaluation of arterial blood gases. It may be obtained directly from the artery (commonly the radial artery) by personnel who are trained to perform this procedure and are knowledgeable about the complications that could occur as a result of this procedure. Arterial blood may also be obtained from a vascular access device (VAD) inserted in an artery such as a femoral arterial line or Swan-Gantz catheter.

It occurs two ways congenitally or as a result of a disease affecting the iris.
The result is decreased function of the iris and pupil.
There are two different types of polycoria, true polycoria and pseudopolycoria.
True polycoria is considered to be when all pupils dilate and contract simultaneously.
Pseudopolycoria is chacterized by the presence of a passive contraction that differentiates the extra and principal pupil.
The incidence is unknown but according to a 2002 article there have only been 2 cases since 1966.
