For @ everyone - military who want to join the LC case (ONLY IF YOUR APPEAL WAS DENIED).
Use the previous post to fill out your questionnaire. Then do the following:
1. Prepare an email
2. Subject line should say something like: Active duty (Branch), stationed at (location), appeal denied
3. Body of the email should list the 5Ws: who: your name, rank, years of service; what: your current status (appeal denied, ordered to take the shot, pending legal, etc.), when: whenever the most recent action occurred against you or your no later than date to take the shot, where: you are currently stationed, why: Provide a chronological record of your situation (including any actions that you’ve taken as a result of the mandate like submission of early retirement requests with the status or the result, other requests for exemption, etc.).
4. Submit a current biography/professional resume if you have one.
5. Provide a paragraph with any comments or actions your command has taken against you. For example, stopping you from taking leave, making you go to TRS, discriminatory acts.
6. Include anything that you may have done to get into trouble regardless of whether or not it is mandate related (they MUST know this information to determine suitability/risk of having you as a plaintiff and how it could affect the case).
7. Add all documentation as attachments to your email to include all original RAR, appeal, other requests, and endorsements.
When you have ALL this information together, and you filled out the form, send your email.
