Okay, as stated above "If you're not a fella, but a lady" means simply that every character has a male gender in game by default.
Using the gender change command works like a trigger and swaps you into a lady (first time free as mentioned in /other, then at a cost of some gems).
Please read the information messages before clicking blue text.
Also, once again - this change does not affect game texts, just the available graphics.
