يعني ايه Canal transportation وايه اسبابه
قبل اي حاجه لازم تعرف ان معظم ال root canals بتبقى curved واي curved canal بيبقي ليها outer convexity و inner concavity
ال canal transportation بقي عبارة عن ان بيحصل straightening لل curved canal بمعنى ال instruments وهيا فى الكانال بتشيل من apical outermost convexity وال middle inner concavity وده بيحصل بسبب ان اى instrument بتحاول ترجع لشكلها ال straight فى الكانال
اسباب ال canal transportation هيا كالاتى:
1- insufficient designed access cavity
بمعنى انك لو مشلتش كفاية من ال axial wall وعملتstraight line access لل canal orifice هيحصل canal transportation
بردو لو معملتش coronal flaring كويس هيحصل canal transportation
2- type of instrument used
بردو نوع ال instrument ممكن يزود فرصة حدوث ال canal transportation:
ال instruments اللي فيها non cutting tip زي معظم ال Rotary NiTi files بتقلل فرصة حدوث ال canal transportation
لو استخدمت stainless Steel files اكبر من size 20 فى curved canals فانت كده بتزود فرصة حدوث ال canal transportation
نوع ال instrument alloy بردو بيفرق
يعني ال NiTi files بتقلل نسبة حدوث ال canal transportation بالذات الانواع الحديثه اللى مصنوعه من
CM-wire & M-wire
بردو ال taper بتاع ال ال instrument بياثر
فكل ماتزود ال taper بالذات لو اكتر من taper 4 كل ما تزود فرصة حدوث ال canal transportation
3- operator experience
معظم ال less experienced dentists بيزودوا ال force على ال files وهما شغالين بالذات مع rotary files فبيزودوا فرصة حدوث ال canal transportation
4- instrumentaion technique
بردو نوع ال technique بيأثر عل فرصة حدوث ال canal transportation
بيقولك ان افضل technique يقدر يمنع ال canal transportation هوا ال balanced force technique
وعموما ال corono-apical techniques بتقلل ال canal transportation عن ال apical-coronal techniques
5-file precurvature
لو معملتش precurvature لل SS files فانت كده بتزود فرصة حدوث ال canal transportation
6- motion of manual instruments
بيقولك ان ال reaming motion بتقلل ال canal transportation عن ال filing motion
بردو مفروض تعمل anticurvature filing motion فى حالات معينه زي مثلا ال mesial canals of lower molars
7- canal anatomy
طبعا كل ما canal curvature يزيد كل ما يزود فرصة حدوث ال canal transportation
8- glide path
بردو لو معملتش glide path فانت كده بتزود فرصة حدوث ال canal transportation

2022-05-10 18:32:21 ملحوظة مهمة جدا من كتاب "Easy endodontics" صفحة رقم ١٨٨
كتير من الدكاترة لما يقوموا بادخال ال file فى ال canal لاول مرة وال file يدخل فى ال canal ماعدا بعض الملليمترات فى الاخر فيظن ان فيه calcification فى ال canal
للاسف فى اغلب الحالات مش بيكون فيه calcification لان الطبيعى ان ال calcification بيبدا coronally ويمتد apically فمش معقول ان ال canal تكون patent فى ال coronal part وبعدين تكون calcified فى ال apical area
الحقيقة السيناريو ده بيكون بسبب حاجة من ال ٣ اسباب:
يكون فيه sudden acute apical curvature (خلى بالك انه مش لازم ال curvature يكون mesiodistally ممكن يكون buccolingually عادى وفى الحالة دى مش هيظهر فى ال conventional periapical radiograph) الحل هنا هتعمل precurvature لل file tip وتخلى ال direction of precurvature مع ال direction الخاص بال curvature اللى فى ال canal
السبب التانى ان يكون فيه accumulated debris فى ال apical third الحل هنا هتعمل file precurvature وتستخدم edtal gel lubricant و copious irrigation
السبب التالت ان يكون فيه coronal binding بمعنى ان ال file بيكون مزنوق فى ال coronal third بسبب انك ما عملتش coronal flaring كويس فالفايل مش بيكون قادر يدخل أكتر فى ال canal الحل هنا هنعمل coronal flaring Open / Comment

2022-04-18 22:40:21 Surgical crown lengthening for UL 5.
Followed by vertical preparation and delayed impression.
Retreatment done as both crowns were connected and PA lesion related to UL 5.
So bridge broken at connector area so we had to remake both crowns but separate this time. Open / Comment
