Indications of splenectomy

TRAUMA

Not all cases require splenectomy, it is indicated only if there is persistent bleeding, or a severely disrupted spleen.

ONCOLOGICAL

In lymphoma, splenectomy has therapeutic role as well as for staging purposes when stage significantly impact treatment options, also splenectomy is a part of en bloc resection of the stomach in gastric malignancy.

HEMATOLOGICAL

In chronic hemolytic anemias the spleen is a site of RBC destruction especially in hereditary spherocytosis, thalassemia and autoimmune Hemolytic Anemias refractory to treatment. Idiopathic thrombocytopenic purpura resistant to treatment is also an indication.

PORTAL HYPERTENSION

Needed for segmental hypertension due to splenic vein obstruction, or as part of shunt surgery.

OTHERS

Massive bulky spleen causing hypersplenism, discomfort, and endangered by trauma can be regarded as an indication.

Above fig shows splenic rupture in a trauma patient, the charactaristic notch is also visible.

Wandering spleen

Also called drifting spleen or floating spleen or splenoptosis, is a condition in which the ligaments that support the spleen are weak or congenitally abscent, this may cause the spleen to descend into the pelvic cavity, figure above shows the anomaly (see black arrow), it is probably not genetic, and also can be acquired due to connective tissue disorder or previous trauma.

The importance is liability for torsion, treatment can be done by splenopexy in which the spleen is fixed, splenectomy is also an options.

24 year old male working at book store presented with left upper quadrant pain following blunt abdominal trauma after getting into a fight with a customer, on examination the patient looks pale and sweating but is responsive with no confusion, on examination the left upper quadrant was tender with guarding, there was left shoulder pain when foot of the bed is elevated, there was no obvious abdominal distension and no shifting dullness, Blood pressure is 110/80 mmHg, pulse rate is 130 beat per minute, respiratory rate is 20 breath per minute, a CT scan was done and showed the above picture, the patient remained to look stable, vital signs are taken regularly with no evidence of decompensation.
