1. If Candidate himself/herself is Covid-19 positive // have suffered from COVID -19 on/after 15th April, 2021 the following are required to be uploaded:

2. If any member(s) of the family (grandparents, parents, spouse, children & siblings) with whom the candidate is staying in the same premises are COVID-19 positive / have suffered from COVID -19 on/after 15th April, 2021:
