You can't just cover the entire surgical field with a single thread. You have to suture everything layer by layer .
Organ, muscle, aponeurosis, fascia, and only then skin (depending on the anatomical area, the wound elements and sequence may vary).
Very often (almost always) in the operated area a drainage is left - a hollow tube, which is taken out through the wound or additional incision.
