ال gingivectomy من أبسط الحاجات اللي ممكن تبدأ بيها شغل ال perio بس لانه هدفه أغلب الوقت Esthetics هتحتاج تعرف شويه قواعد بسيطه علشانً نظبط الgingival margin scalloping خصوصا في حالات :
1) Gingival hyperplasia . 2) Altered passive eruption.... causing gummy smile.... needs esthetic crown lengthening . 3) Short clinical crown and in need for lengthening to avoid compromising the periodontium .... needs Functional crown lengthening.
-في الحالات اللي ذكرناها دي حضرتكً هنحتاج تعمل gingivectomy or modified wideman flap فلازم تعرف gingival zenith لكل tooth ببقي فين ؟!
-طَيب هو ايه gingival zenith ؟ -It is the most apical point of the gingival margine and by placing it in its right position , you can achieve the proper esthetic result that you need.
*Let's determine the gingival zenith for each tooth in relation to the verticlal long axis bisecting the crown: -------------------------------------
~For the upper central incisor : 1mm distal.
~For the upper lateral incisor : centralized.
~For the upper canine : 1mm distal or centralized according to the situation.
~For the rest of the upper teeth "premolars and molars ": centralized
~For the lower teeth "anteriors and posteriors" : centralized
# What about the gingival zenith level for each tooth and others ?!!
~The gingival zenith of upper central incisor and canine are at the same level but the lateral incisor is 1mm coronal to it !
