Did your command push an applicant information form out to be submitted as an enclosure to the appeal? I’ve heard of some filing EO complaints to have this requirement removed from the process. Anyone else dealt with this?

Yes some people are seeing those. File a complaint because it is not a part of the MCO process and them using it without giving you an opportunity to rebut it in your appeal is inappropriate.
