Despite high efficacy and few adverse events found in clinical trials, only 56% of individuals in the US reported wanting to receive the vaccine. One of the reasons for vaccine hesitancy is the potential negative effect on fertility.

In this study of sperm parameters before and after 2 doses of a COVID-19 mRNA vaccine, there were no significant decreases in any sperm parameter among this small cohort of healthy men.

● Myopericarditis occurred in 13 teens (median age, 15 years; 12 male) after the second dose of COVID-19 mRNA vaccine.

● Ten teens were non-Hispanic white, two were Asian, and one was American Indian/Alaska Native.

● Symptoms developed a median of 3 days after second vaccination.

● All received nonsteroidal anti-inflammatory medications. Two patients with LV abnormalities received corticosteroids and intravenous immunoglobulin.

● Median hospital stay was 2 days; no patient required intensive care.

● All patients survived and improved without further morbidity prior to discharge.

The B.1.617.2 (delta) Covid-19 variant has surged in India and spread worldwide. In a test-negative case–control study, the effectiveness of two doses of BNT162b2 (Pfizer) was 94% against the B.1.1.7 (alpha) variant and 88% against delta; with the ChAdOx1 nCoV-19 (Oxford, AstraZeneca) vaccine, effectiveness was 74% and 67%, respectively. Protection after a single vaccine injection was low; two doses are needed.

Findings - In this case series of 23 male patients, including 22 previously healthy military members, myocarditis was identified within 4 days of receipt of a COVID-19 vaccine. For most patients (n = 20), the diagnosis was made after the second dose of mRNA COVID-19 vaccine; these episodes occurred against the backdrop of 2.8 million doses of mRNA COVID-19 vaccines administered.

Meaning - Vigilance for rare adverse events, including myocarditis, after COVID-19 vaccination is warranted but should not diminish overall confidence in vaccination during the current pandemic.
